  • Relaxed visits with the doctor. Eight hour-long visits per year are included; additional visits are $300

     

  • Unlimited phone calls, emails, and texts with quick responses during working hours

     

  • In-depth review of your personal and family medical histories

  • Comprehensive, customized plans to support your health

     

  • Pro-active genetic testing for hereditary cancers

Fees &

HOW MEMBERSHIP WORKS

  • A Joining Fee of $3,000 covers your on-boarding process into the practice AND our first 3 months of unlimited work together. This includes:

     

    • Extremely detailed intake visit(s) and follow ups, as many as needed

       

    • Thorough records review from previous health care experiences

       

    • Coordination with other doctors and specialists

       

    • Ordering and interpreting detailed labs and other studies (like sleep studies or imaging)

       

    • A comprehensive physical exam

       

    • Communication in between visits – phone calls, emails, texts

       

  • After our first 3 months of unlimited care together, you may continue as a member for $250 per month (adults and teens age 13 and up). The monthly membership includes 8, hour-long office visits per year. Additional visits are $300.

     

  • Cancel your membership with 30 days’ notice for any reason

     

  • Your insurance still covers your medications, most labs, imaging, specialist care, and hospital care

     

  • Membership fee does not accept Health Savings Accounts (HSAs), insurance plans or health sharing programs at this time.

     

Appointments:

New patients are scheduled for 90 minutes; return visits are 30 to 60 minutes (your choice). Members can easily schedule online. Appointments are unlimited for the first 3 months. Thereafter, your membership includes 8 hour-long visits per year. Additional visits are $300.
